---Programs in English: TOEFL/IELTS/other National certificate (for non-native English speaker)

6. Non-criminal record certificate

7. Foreigner Physical Examination Record and Blood Examination Report

8. Personal Statement (Less than 800 words; including previous study and work experience, reasons for application, study plan, etc.)
