*1. Application Form

2. Applicants who are under the age of 18 by September 1, 2022 must submit
Guardian guarantee and the guardian must be an adult living in Changchun.
*3. Foreigner Physical Examination Form
Complete in English “Foreigner Physical Examination Form” (The form is uniformly designed and printed by the Chinese Health and Quarantine Department.) Applicants should strictly follow the requirements in the checklist and cover all the inspection items. Incomplete records, no sealed photograph, no signatures of the attending physician or the official stamp of the hospital are invalid. The inspection result is valid for 6 months.
*4. Transcripts of all high school courses
Academic transcript in languages other than Chinese or English should be translated into Chinese or English and be certified by notarization.
*5. Certificate of deposit
6. Those who were originally Chinese citizens and then joined foreign citizenship, if both or one of their parents still possess Chinese nationality, need to submit:
(1) Proof of having foreign nationalities for more than 4 years;
(2) Passport entry and exit records from April 30th, 2018 to April 30th, 2022;
